Why do you get headaches from tension in your neck and shoulders?

Tension in your neck and shoulders can cause headaches due to the close connection between neck muscles and the nervous system in your head. Tense muscles can pinch nerves and affect blood supply to your head. This cervicogenic headache literally originates from your cervical spine.

The cervical spine has direct nerve connections to your head via the occipital nervous system. When the upper cervical vertebrae do not function properly or surrounding muscles are tense, they can irritate these nerves. This results in pain that radiates to your back of the head, temples or even your forehead.

Poor posture, especially protruding your head forward during computer work, places extreme strain on the neck muscles. This posture is also called “forward head posture” and can lead to chronic muscle tension. The muscles must constantly work extra hard to keep your head in position.

What is the difference between tension headaches and migraine?

Tension headaches feel like a tight band around your head with mild to moderate pain, while migraine is characterised by intense, throbbing pain, often on one side. Migraine is usually accompanied by nausea, light sensitivity and can last for hours. The triggers and treatment also differ significantly.

Tension headaches are the most common form and usually arise from muscle tension, stress or fatigue. The pain is usually pressing or constricting, as if there is a too-tight hat around your head. This headache usually does not seriously affect your daily activities and responds well to ordinary painkillers.

Migraine, on the other hand, is a neurological condition with more complex symptoms. The pain is often throbbing or pounding and gets worse with movement. Many people experience an “aura” preceding the headache, such as visual disturbances or tingling. Nausea from headaches occurs much more frequently with migraine than with tension headaches.

The triggers also differ significantly. Tension headaches are usually triggered by stress, sleep deprivation or poor posture. Migraine can be triggered by hormonal changes, certain foods, weather changes or strong odours. Women get migraine more often, often related to their menstrual cycle.

The side effects of headaches are also more severe with migraine, with symptoms such as vomiting, extreme light sensitivity and the need to rest in a dark, quiet room.
